Still, no Briton is ever entirely happy with the taxpayer-funded service, and now the case of a 5-year-old boy with a brain tumor has thrown a harsh light on the $170 billion-a-year system.
Properly described in the second paragraph, so contrasting the appearance of the NHS to the typical patient with the appearance to the boy with cancer deep into the story is not egregious. The author knew and recited that it is an expensive taxpayer funded system at the very beginning.

I guess the part about “for the most part, it’s services are free” confused me. I keep forgetting who the NYT’s audience is.
